The Widal Test by Tube Method is a semi-quantitative serological test that detects agglutinating antibodies against the somatic (O) and flagellar (H) antigens of Salmonella typhi and Salmonella paratyphi A & B in a patient's serum. Unlike the slide method, the tube dilution technique provides titres, allowing assessment of disease severity and response to treatment. A rising titre over serial samples (4-fold or greater increase) is diagnostically significant. A single titre of 1:160 or above for O antigen is generally considered positive in endemic areas. The test is recommended for patients with prolonged fever (more than 5 days), abdominal pain, and relative bradycardia. Blood culture remains the gold standard; Widal is used as supportive evidence.
